via fax or mail:

do NOT accept
phone registration.
- You will need to have Adobe
Reader to view these forms.
- One Form Per Child.
- Please include the session number or dates
of the program you are requesting.
- We are unable to process incomplete registration forms. Forms will
be returned to you and will not be considered until a completed
form and payment has been returned.
